Player Development Plan :

The Canongate Golf Performance Center Membership offers Players the following benefits until cancelled by either the Player or The Performance Center. In consideration for the annual agreement, the Player and any included family members listed below shall have the following privileges:

v Professional Individual Instruction starting at fifteen dollars ($15.00) per ½ hour lesson, thirty dollars ($30.00) per one hour lesson.

o Limited to once per week per membership

o Lessons may be booked one week in advance and are based on availability

v Discounts on Performance services (see Performance Center pricing sheet for current member rates), invitations to special golf events, priority access to select clinics and tournaments.

v Complimentary use of the driving range at Canongate facilities.*

The Performance Center Membership is available for the Player and, for an additional fee, family members. Upon execution of the PDP Agreement and payment of the Plan Fee indicated below, the Player and named family members shall receive access to the Performance Center Membership privileges.

Payment: The plan is an annual agreement and will automatically renew unless written notification is given 30 days prior to the end of the agreement. All new members are required to utilize EZ-Pay. This system allows you to automatically pay your monthly fee and club charges with your credit card. This system is worry free as you will always receive a detailed statement via mail and email prior to your credit card being charged. To enroll in EZ-Pay via credit card, simply enter your credit card information below.

I agree to immediately report to the Club all changes to the above credit card information. I agree that the Club is not responsible for any charges or expenses (e.g. for overdrawn accounts, exceeding credit card limits, etc.) resulting from charges billed by the Club. Children under 16 must be signed in by a parent or guardian and children 11 and under shall be accompanied by a parent or guardian while on property. Soft spikes and proper golf attire required (collared shirts and no denim).

I agree to remain a member for at least one year after which time I can resign by providing the club with 30 days written notice. I understand that I am responsible for all dues and applicable fees during the 30 day notice period. If I should resign before one year, I agree to pay a $100 cancellation fee.
